The Audiology Center of Redlands, Medical Clinic, Inc,,
agrees to act as Hearing and Noise Consultants and Representatives
for the City of Redlands and will provide professional services,
at the option of the City, according to this fee schedule:
1. Pre -employment pure -tone audiograms including pertinent
employee history of noise exposure and medical problems
associated with hearing. Fee: $15.00 per audiogram.
2. Consultation services including but not necessarily limited
to the planning and/or execution of hearing conservation
and noise measurement and control programs, educational
seminars, etc. Fee: $35.00 per hour.
3. Additional diagnostic services as needed. Fees will be
determined on the basis of services rendered in individual
cases and will follow the regular schedule of professional
fees for private patients of the Audiology Center.
4. Specialized equipment may be rented or leased to the City
of Redlands for implementation of hearing conservation or
noise measurement and control programs. Fees will be
determined on the basis of individual pieces of equipment
and their use.
5. Original records of pre -employment audiograms will be kept
at the Audiology Center and copies of these records will be
sent to the City of Redlands for the employees' personnel
file.
6. The Audiology Center will bill the City of Redlands for
professional services rendered on a monthly basis.
7. The Audiology Center or the City of Redlands may terminate
this agreement at any time.
