HomeMy WebLinkAboutContracts & Agreements_40-1999_CCv0001.pdf ura R VE GRANT AGREEMENT 663B-8827-G3615 BETWEEN PUBLIC HEALTH INSTITUTE/ CALIFORNIA HEALTHY CITIES AND COMMUNITIES AND CITY OF REDLANDS a i The Public Health Institute is the recipient of Contract#96-26044 from the California Department of Health and Human Services for conduct of the "California Healthy Cities and Communities" (PHI/CHCC) program for promotion and implementation of health and preventive services in California communities, including acquiring technical assistance in conducting their Healthy Cities efforts. PHI/CHCC and City of Redlands, P.O. Box 3005, Redlands, CA 92373, (909) 798-7545, FAX (909) 798-7648, hereinafter GRANTEE, EIN 95-6000766, agree as follows, effective on the subscribed date: PURPOSE OF GRANT: GRANTEE agrees to use the funds provided by this grant exclusively for the purpose of acquiring technical assistance services for use in conducting its 1998/99 Healthy Cities and Communities activities as set forth in its 1998/99 Healthy Cities and Communities application to PHI/CHCC, which is included herein by reference. Technical assistance services may include obtaining the personal service of consultants and student interns, purchasing educational or promotional materials, obtaining professional design or marketing services, and similar matters. TERM: Funds under this grant shall be available for expenditure from July 1, 1999 through June 30, 1999. PAYMENT: Upon receipt of a fully-executed grant agreement from GRANTEE, PHI/CHCC will release the full grant award amount of$3,000.00. Expenditures shall conform with Attachment A. GRANTEE may modify the budget with prior written approval from PHI/CHCC, but no such modification shall increase the total amount. ACKNOWLEDGMENT: GRANTEE shall acknowledge as follows the support of the State of California whenever any findings, data, or materials acquired under this grant are used in any publications: "This material is based upon work supported by the State of California, Department of Health Services under Contract#96-26044." PROGRESS AND FISCAL REPORTING: GRANTEE shall provide interim progress reports as required by PHI/CHCC and a final progress report on or before July 31, 1999. GRANTEE shall provide a final fiscal report detailing its expenditures on or before July 31, 1999. GRANTEE shall promptly return the unexpended balance, if any, of funds awarded under this grant agreement. Equipment and unused supplies can either be used by GRANTEE for future Healthy Cities and Communities activities funded by PHI/CHCC or returned to PHI/CHCC. AUDIT: GRANTEE agrees to maintain and preserve until June 30, 2002 and to permit PHI/CHCC and the State or any of its duly authorized representatives to have access to and examine and audit, any pertinent books, documents, papers, and records of GRANTEE. Skills and Training needed in using e-mail. • No e-mail address. II. COMMUNITY BUILDING ENHANCEMENT OPTIONS Option A• Electronic Communication Technology Our city would utilize this grant to: (1) Establish Internet Access - Purchase (1) computer, modem, and electronic networking services, including e-mail, (2) Utilize this tecnology to share information and resources throughout our city and East Valley community and contribute to cross-learning among California Healthy Cities and the international movement. COST: $2,492.00 Option B• Team Building/Leadership Development Our city would utilize this grant to: (1) Fund the participation of primary participants in our Healthy Cities Youth Coalition to attend the California Healthy Cities and Communities Annual Conference. The grant win be used for registration fees, travel and lodging. (2) By having a team attend the conference, our expectation is their understanding of the healty cities and communities approach will increase and their community building skills will improve as a result of participating in the conference.
