This is Amendment No. 1 to the Agreement between Verizon California Inc. (Verizon), located at 112 Lakeview Canyon
Road, Thousand Oaks, CA 91362 and Customer for the provision of Transparent LAN Service (TLS) (the Service), which
was effective March 14, 2014.
WHEREAS, Customer wishes to purchase additional Service,
THEREFORE, the Parties agree to amend the Agreement as follows:
1. Consistent with Section 1.3.1 of Exhibit A to the Agreement, Customer may purchase the following additional TLS
Ethernet Multipoint Service (EMS) Standard Access Lines in the Quantities and at the bandwidths and locations
indicated below pursuant to the rates, terms and conditions of the Agreement. Non-recurring installation charges
are not applicable to such additional Services.

2. Except as amended herein, all provisions of the Agreement, including Exhibit A thereto, shall remain in full force
and effect.
3. This Amendment No. 1 shall be effective when executed by both parties (the Effective Date).
4. Customer must execute this Amendment No. 1 no later than July 1, 2014, or the proposed Service arrangements
and pricing will no longer be available.
The parties have caused this Amendment No. 1 to be executed by their respective duly authorized representatives.
