We are pleased to present the renewal quotation from Safetly National Casualty Company for the
City's Excess Workers Compensation Insurance. Terms are minimum/deposit premium of
$135,668 for $25,000,000 maximum limit of indemnity per occurrence subject to $1,000,000 self
Insured Retention per occurrence. Premium rate is $0.4051 per$100 of payroll estimated at
$33,999,999.
Hub International Insurance Services Inc is hereby authorized to bind and renew coverage.
