No. 1 Page 1 of 2 PUBLIC WORKS DEPARTMENT CHANGE ORDER PROJECT TITLE: COMMUNITY CENTER RENOVATIONS, PHASE III CONTRACT No. 23-6810-2960 CONTRACT DATE: NOVEMBER 16, 1999 CONTRACTOR: NBI GENERAL CONTRACTORS The following changes are hereby made to the Contract Documents: - Increase contract amount by $1,500.00 to replace surface-mounted lights in 4 restrooms. - Increase contract amount by $4,853.00 to remove existing partitions and replace with steel partitions with baked enamel finish, including toilet accessories. - Increase contract amount by $2,250.00 to demolish partitions, wing walls and wall tile, including the replacement of drywall and all patch work. - Increase contract amount by $6,900.00 to install approximately 2,600 square feet of FRP board. - Increase contract amount by $7,100.00 to replace all plumbing fixtures in 4 restrooms. - Increase contract amount by $3,000.00 to install new Steelcraft doors with continuous hinges at Game Room No. 1 and Arts & Crafts Room, not including panic hardware, if required. - Increase contract amount by $6,400.00 to cover Overhead and Profit for additional work outside original contract scope. - Increase contract amount by $750.00 to cover Bond expenses for additional work outside original contract scope. Justification: Additional work was requested by the Community Services Division/Recreation Bureau of the Police Department. This work will complete the 4 public restrooms within the building, bringing 2 of the restrooms into compliance with all handicap accessibility codes.
