Health and fitness are emotional subjects. There is a universal recognition of the "feeling"one gets during and after exercise. People who are fit and healthy look and act differently—more confident, less stressed and more energetic. By implementing the 24 Hour Fitness corporate program you will provide the opportunity for your employees to feel better, increase productivity and miss less work time, resulting in faithful, long term employees. What a valuable investment! You will benefit from a reduction in insurance and compensation costs and address fierce competition for the retention and recruiting of talented employees. In an era of rising healthcare costs, a corporate fitness program is essential to the economic livelihood of a business. 24 Hour Fitness is the world leading provider of fitness services and corporate fitness programs. We have the locations, professionalism,customer focus, and experience to be your long-term partner. Upon receipt of your payment in the amount of$2,500 plus applicable tax (which represents the 2002 Renewal Sponsorship Fee for 250 up to 499 employees ),a signed copy of this agreement, and official confirmation of the total number of eligible employees from City of Redlands, 24 Hour Fitness will activate your corporate rate and administer an annual open enrollment period for your employee population. Your 24 Hour Fitness corporate representative will assist you in the promotion of this open enrollment period and the organization of on-site enrollment presentations. 24 Hour Fitness will provide you with approved E-mail flyers or other acceptable promotional materials to ensure maximum participation. Throughout the open enrollment period,your employees can take advantage of one of our Keep Fit All-club membership programs at the rates listed below. These memberships are mouth to month, pay as you go (EFT), stop when you want with no hidden costs or long term commitment. There is no minimum term or cancellation fees and your employees can stop their dues at any time by filling out a"request to stop dues"form at any 24 Hour Fitness location. This investment waives the enrollment fees associated with our Keep Fit membership programs for your employees at the above location only and guarantees the following monthly dues rate: Rates for Southern California(Redlands Area) Emplovees Fitness Sport Single: $0 Enrollment, $19 dues/month $0 Enrollment, $29 dues/month Couple : $50 Enrollment, $29 dues/month $50 Enrollment, $39 dues/month Familv of 3: $90 Enrollment, $39 dues/month $90 Enrollment, $49 dues/month Each Add-On: $0 Enrollment, $10 addt'l dues $0 Enrollment, $10 addt'l dues This agreement begins on the date below and expires one year from that date. As an added benefit, new hire may enroll at the same rates during this agreement and must provide proof of employment upon enrollment. For those employees who are current members with"Keep Fit"memberships, they may reduce their dues to the corporate rate at an time during the open enrollment periods by providing proof of employment and filling out an"EFT Change Form"at any Hour Fitness location. Finally, our"corporate renewal program"makes it very easy for you to continue with this program on an annual basis provided you renew no later than 30 days after the expiration of this agreement. Our 100%renewal option allows y, to continue to offer the same low dues rate for the upcoming year. Our 50%option allows you to continue to offer this great benefit but at the current dues rate.
