ACTION
CONTRACT AGREEMENT BETWEEN
THE CITY OF REDLANDS AND COMMUNITY ACTION/EAP
The City of Redlands is renewing its contract with Community
Action EAP, effective September 1, 1996, with the inclusion of
the following contract termination clause and the City's
selection of program cost option based on eligible program
participants as specified by the City of Redlands.
CONTRACT TERMINATION PROVISION
This agreement may be terminated by either party upon thirty
(30) days prior written notice to the other party for any
reason whatsoever, and once such notice has been given, CA/EAP
shall not begin any assessment services for any employee's new
problem or for any employee who had not previously been
evaluated pursuant to this agreement. Upon reception of such
notification, program costs shall cease to accrue, and the
payment for the month in which the termination occurs shall be
prorated appropriately. Assessment and evaluation of
employees which had begun before notice of termination shall
be continued on a case-by-case basis until completed or
referred to outside health care professionals.

PROGRAM COST ADDENDUM
OPTION 1: The monthly administrative fee covering all employees
and their family members, with inclusion in the existing
EAP service contract with Community Action EAP of Police
and Fire Employee Association members and their family
members, is $ 920.00 for which CA/EAP will bill the City
of Redlands monthly.
Option 2: The monthly administrative fee for EAP services covering
employees and family members, excluding Police and Fire
Employee Association members and their family members
from said services, is $ 590-00 for which '.-'A/EAP will
bill the City of Redlands monthly.
