FOR THE PROVISION OF SPAY AND NEUTER SERVICES
This first amendment ("First Amendment") to the agreement to provide spay and neuter
services dated June 23, 2015 (the"Agreement"), by and between the City of Redlands, a municipal
corporation ("City"), and Highland Village Pet Hospital ("Contractor"), is made and entered into
this 16'h day of February, 2016.
RECITALS
WHEREAS, it is the desire of City and Consultant to amend their existing Agreement,
specifically Paragraph 2.1 and Exhibit"A"to the Agreement relating to"Services of Contractor;"
NOW,THEREFORE,City and Contractor agree as follows:
AGREEMENT
Section 1. Paragraph 2.1 of the Agreement, entitled "Service of Contractor" is hereby
amended to read as follows:
2.1 The specific Services that Contractor shall perform are to provide veterinary services
and medical supplies, including spay/neutering and associated services, on an as
needed and requested basis by City and as more particularly described in Exhibit
"A,"which is attached hereto and incorporated herein by reference.
Section 2. Exhibit"A"to the Agreement, entitled"Services of Contractor,"is hereby
deleted and a new"Services of Contractor"Exhibit is added in the form attached hereto as Exhibit
«A
Section 3. All other provisions of the Agreement shall remain unchanged by the First
Amendment, and in effect.
IN WITNESS WHEREOF,the parties have executed this Fust Amendment,to be effective as of
February 16, 2016.

HIGHLAND VET
SCOPE OF SERVICES
Fee proposal structure:
Class One:
General Veterinary Services
1. Outright, fixed cost of veterinary sei vices per 12 months: $1_,200 (*$100/month)
2. Vet Technician., 5 days per week @ 5 hours per day: SI5lhr.=(Hourly rate x 25)
3. Mark up for medical supplies 2% over cost
4. Price list of additional services offered:
(For example: Medical cases such as broken limb pain management,broken Iimb treatment,
evaluation for euthanasia,medical grooming,general grooming,etc.)
Class Two:
Spays and Neuters
1. Cost per Spay—K-9
0-30 lbs. $60 31-50 lbs. $70 51-75 lbs. $85 76-100 lbs. $110
2. Cost per Neuter—K-9
0-30 lbs. $55 31-50 lbs. $62 51-75 lbs. $72 76-100 lbs. $82
3. Cost per Spay—Feline: $20
4. Cost per Neuter—Feline $15
5. Additional charges if animal is in heat/pregnant or has retained a testicle. (if applicable):
Not to exceed$250 per procedure.
